QBC ParaLens Advance™ Evaluation Agreement

I (the customer) agree to receive from Drucker Diagnostics, Inc. a QBC ParaLens Advance complete evaluation system for a 30-day evaluation period, for the purpose of evaluating the system for purchase, pursuant to the following terms and conditions:

Evaluation Period

The 30-day evaluation period begins on the date of delivery of the evaluation system. Under certain circumstances, Drucker Diagnostics may offer an extension to this period. Requests for extension must be submitted in writing (including email) before the expiration of the evaluation period, and approved by Drucker Diagnostics to be considered valid.

Complete Evaluation System

The complete evaluation system consists of the following components: LED light source, main body assembly(s) (including objectives), filter set arm, power pack, international plug adapters, and carrying case.

Purchase Option

At or before the end of the 30 day evaluation period, the evaluation system may be purchased directly from Drucker Diagnostics. The evaluation system  will be eligible for the full manufacturers warranty from the date of purchase.

Return of Evaluation System

The complete evaluation system may be returned prior to or no less than the last day of the evaluation period to Drucker Diagnostics without cost or obligation. To return the evaluation system, a Customer Service representative should be contacted (877-231-3115 or 814-692-7661) to obtain a Returned Goods Authorization (RGA) number and a Call Tag for shipment by the preferred carrier of Drucker Diagnostics.

Shipping Costs

Drucker Diagnostics agrees to pay all costs associated with shipping the evaluation system to and from the customer site, provided that the customer adheres to all requirements of this agreement. If the customer fails to adhere to the terms of this agreement, he or she assumes responsibility for shipment of the evaluation system to Drucker Diagnostics.

Failure to Return Complete System

If the complete evaluation system is not returned to Drucker Diagnostics within one week of the conclusion of the 30-day evaluation period, Drucker Diagnostics reserves the right to automatically convert this evaluation agreement to a purchase order at the full retail value of the evaluation system.

Missing Components

If any of the components of the complete system, with the exception of consumables (herein defined as immersion oil and the preventative maintenance kit) are not returned prior to or at the end of the evaluation period, Drucker Diagnostics will consider this a failure to return the complete system and reserves the right to invoice the customer at the full retail value of the missing component(s).

Damage to System

The customer assumes responsibility for any damage caused to the evaluation system in any capacity other than normal use. This may include, but is not limited to: dropping system components, getting the LED light source or power pack wet, operating the system in a manner not specified by the operators manual, or attempted repair not authorized by Drucker Diagnostics.

Limitation of Liability

Drucker Diagnostics disavows all liability for any incidental, direct, indirect, punitive, actual, consequential, special, exemplary, or other damages, including loses of revenue or income, pain and suffering, emotional distress, or similar damages arising out of any malfunction of the evaluation system during the evaluation period.

Survey

By participating in the 30-day evaluation program, customer agrees to participate in a post-evaluation survey to be distributed at the discretion of Drucker Diagnostics. Customer agrees that survey comments are the exclusive property of Drucker Diagnostics, and may be used in marketing materials without prior approval.

Geographic Limitation

This offer is only valid in the United States and Canada.
